Saint Elizabeth Community SASH Coordinator in East Greenwich, Rhode Island


The SASH (Support and Services at Home) Coordinator is responsible for coordinating care and services for elderly residents of a subsidized housing high rise in Providence, and for convening a multi-agency team to support the implementation of a person-centered model to assist the participants in remaining at home. The SASH Coordinator will build trusting relationships with participants based on a thorough assessment of each person’s strengths and challenges as they pertain to remaining safely in his or her home, and based on respect for each participant’s individuality.


Coordinates transition care as participant enters or exits hospital or other care facility; Provides education in self-care to participants via individual and group delivery models; Coordinates the development of Individual and Community Healthy Living Plans in collaboration with the SASH team; Convenes and coordinates SASH team meetings; Participates in outreach within the residence and to the larger community; Works independently and as part of the SASH team, upholding high standards of care and ethics. SPECIFIC REQUIREMENTS

Minimum BSW or equivalent combination of background and experience. Previous experience coordinating services for seniors and/or adults with disabilities is preferred; knowledge of area resources is preferred. Required: Excellent verbal and written communication skills; respect for culturally diverse populations; driver’s license. Must be Bilingual English/Spanish.
